Gangrene occurs randomly due to the presence of bleeding wounds. Tended wounds have a lower chance of contracting gangrene, but are not complete protection. Gangrene due to normal wounds will not occur until at least 9 minutes after the wound is inflicted. During the course of gangrene it will turn scars into wounds.
