Smelting, unlike all other forms of crafting, cannot produce a finished ingot with a higher quality than its ingredients. When multiple ingredients are used, a perfect success simply averages them out. Storebought ore is of storebought quality (70ish), so refining is always needed to make high quality ingots.

Ingredients and tools do more than just give a skill bonus/penalty. If you have perfect skill and perfect tools, a poor quality ingot will limit how high of a quality the finished product can be - in addition to penalizing your skill. With storebought (undamaged) tools and stroebought ingredients, you can just barely make masterwork items. So, it is always wise to invest in better tools and ingredients when you can afford them. Right now it is a bit hard because all of the other systems aren't out yet, but that will change in time.

Last thing - the crucible takes damage with each use. Make sure to keep it in good repair by pouring the aerated salts into it. A crudded up crucible will prevent you from making high quality finished ingots.
