If Player A kills Player B, our default assumption is that there is some form of consent unless we have a reason to believe otherwise. A player REPORTing is, classically, the most common reason, but on-duty GMs are not restricted to just letting something stand because nobody complained if it's clear the situation isn't kosher (such as if you're killing a noobler who might not know how to complain).
For better or worse, PvP policy involves a generous helping of human decision making and trying to codify it down to "GM will do this" or "GM will not do that" generalizations is just going to get you blindsided. | |
