The conversion of wounds to scars has never been consistent. In fact, if you do some serious testing on EWH or even better, IWH, you'll find the that it can get very strange indeed.

There are two basic problems.

The first is that wound-to-scar conversion is handled at the engine level, so GMs have no direct control over the actual process, although I suppose we could fake it by heavy number juggling and manual wounding/healing to recalibrate. The engine does have a fairly consistent method of handling the conversion, but it can lead to what appears to be inconsistent results if you are looking for the wrong thing. I know this is counter intuitive, but the best way to minimize the scar tissue is to heal your wounds over multiple casts, the more the better. If you try to heal the entire wound at once, you will get maximum scar tissue.

The second problem is that the wound-to-scar conversion does not know how to handle excess healing. That is to say, if I try to heal 30 points damage and only 20 exist, it messes up the wound-to-scar conversion and the results are not at all predictable. This is what was happening with the old EWH/IWH, because all they did was look at your mana, determine how much you could heal, and apply it, even if you didn't need that much. The result was a rather messy wound-to-scar conversion which was very much inconsistent and did not fit any logical or even rational scheme.

If you play around with HW/HS and use specific targeting (ie ext/int only) so that you are in full control of what you are looking at, I suspect there are people out there than can fairly quickly figure out how wound-to-scar conversion works and how to minimize scars. I'll even give you a fairly direct hint -- it's all about the stacking, or lack thereof.
